Sometimes patients need home care following treatment at the University Medical Center Freiburg.

We want to ensure that you will receive the best possible care after discharge. For this reason, the University Medical Center Freiburg, in conjunction with the own ambulatory home care service PflegeNetz GmbH) and with regional ambulatory home care services established the Regional Home Care Association (Regionaler Pflegeverbund) of the University Medical Center Freiburg. The association serves the districts of Breisgau-Hochschwarzwald and Emmendingen (an area covering approximately 50 km around Freiburg) and ensures an uninterrupted transition from the University Medical Center to the ambulatory home setting.

The objectives of the Regional Home Care Association (Regionaler Pflegeverbund) at the University Medical Service Freiburg is to facilitate your return home as soon as possible and to provide the optimal conditions for your recuperation through high quality care based on customised concepts and established standards.

What does ambulatory home care mean and what does it entail?

The University Medical Center Freiburg often treats patients for acute illnesses. After the treatment of the acute condition, when the patient is ready for discharge from the hospital, the ambulatory care services will take over responsibility and provide you with your home care.

• Due to close cooperation with the University Medical Center, your care may be continued without interruption.
• Certified dedicated personnel will assume your care and train and consult with you and your relatives to ensure your recovery and to further your independence.
• We closely cooperate with your primary care physician.

• Domestic aid, if you wish, can be provided to take care of the household, cook, clean and shop for you.
• We will gladly assist you with your health insurance formalities.
• We serve not only elderly individuals but also young people who require care and support.

How do I obtain ambulatory home care?

If during your treatment in the University Medical Center, it is established that you will subsequently need home care, our staff will take the necessary steps to obtain this service for you. The personnel of the Care Transition (Pflegeüberleitung) Department will contact you and will establish a customised solution for your situation. Naturally, we take your wishes into consideration including your choice of the care provider.

How is the ambulatory home care financed?

If ambulatory home care is medically needed, the costs of the service will be covered by your health insurance. However, there may be costs that are not covered by your insurance for which you must assume responsiblity. The Care Transition (Pflegeüberleitung) Department of the University Medical Center Freiburg and the social centers and deaconries will verify the services and items covered by your health insurance plan. They will then inform you about the scope of services that you are entitled. The consultation and organisation of the care is done by the Care Transition Department without charge.
