



The Health Network at the University Medical Center
The changing conditions within today's health care sector necessitate close cooperation between the University Medical Center and other health care providers. The Health Network at the University Medical Center was established to ensure comprehensive, modern, patient-oriented and convenient health care.
The Regional Rehabilitation Network at the University Medical Center is an association of 14 regional rehabilitation centers and the University Medical Center. The Regional Care Association at the University Medical Center is an alliance between the University Medical Center and more than 20 ambulant maintenance services in the region. The Care Association is supported by the University Medical Center's Discharge Management staff which consists of Social Services (Sozialdienst), the palliative care (Brückenpflege), the Transition Care (Überleitungspflege) and the Patientenring (Palliative Aftercare Service) Departments, optimises treatment processes by coordinating transfer and facilitating communication between the University Medical Center and aftercare facilities and providers.
Due to the large distances from which our patients come from, the University Medical Center Freiburg, coordinate services with 15 surrounding academic hospitals. This close cooperation ensures the highest care for our patients. Additional details regarding academic hospitals are presented separately.
The University Medical Center Freiburg also work closely with office-based private physicians. A large number of these practitioners also have admitting privileges and are able to electronically exchange data with the University Medical Center. Furthermore, a close cooperation has been established in form of the Emergency Practice (Notfallpraxis) for adults in the University Medical Center and is covered by the Association of Statutory Health Insurance Physicians.
