Patient's Guide - Your Stay at the Hospital - After Your Stay
Additional Payment / Co-Payment
Patients who are insured under a public health insurance plan must pay a portion of the cost of their hospitalisation. The University Medical Center Freiburg is legally bound to charge a co-payment of € 10 per day on behalf of the health insurance companies. This amount will be charged for a maximum of 28 days in one calendar year. If you have already been treated in another hospital as an inpatient, these days will be counted. Patients under 18 years of age and patients who are legally exempt will not have to pay this co-payment.

Care Network / PflegeNetz GmbH
If you have medical concerns after your discharge from the hospital and would like to have assistance from a qualified health caregiver, the University Medical Center Freiburg's Care Network, an outpatient service, is available for patients who reside in the Freiburg area.
The Care Network provides comprehensive individualized care. If you are interested in this service, please ask our nursing staff. A brochure describing this outpatient service can be supplied to you by our nursing team. You can also contact the Care Network by phone at +49 761 270 61510.

Care Transfer
For patients who need additional care after discharge from the hospital, a transfer of care can be organised. In consultation with you and your relatives, the staff from the Care Transfer Department can prepare your return home as well as advise and support you with any questions you might have concerning further care.
If you were previously treated as an outpatient prior to your hospital admission, please inform us promptly, so that we can arrange for the resumption of this care in time for your discharge.

Cashier
Our cashier is located in the Administration building (Verwaltungsgebaeude) at Hugstetter Strasse 49. Opening hours are Monday to Friday 8.00 – 12.00.

Discharge
When you have completed your treatment in the hospital, you will be discharged home or to an extended care facility. The attending physician and you will decide on the date of your discharge. If further medical care is needed, you will receive a letter of discharge or a letter will be sent to your private physician and the hospital staff will make arrangements for follow-up care. To facilitate the discharge procedure, the hospital works with a variety of services.

Patientenring GmbH
Following your inpatient stay or outpatient treatment, you may need special medical supplies and equipments. The staff of the Patientenring GmbH coordinates the care that you will need at home and brings to you medical items such as dressing bandages, infusion therapies, oxygen tanks, etc. so that your medical requirements are without interruption. Besides the small co-payment that is necessary for all prescriptions, this service is provided free of charge.

Physiotherapy
If you require or wish for physiotherapy after your hospital stay, you can continue this therapy as an outpatient at our Physiotherapy Center.

Records of Patients / Medical Archive
After your hospitalisation, the hospital forwards a complete medical report or a short report of your medical treatment to your primary care or referring physician, so that your follow-up care and treatment occur smoothly. If you wish, it is also possible to send your hospitalisation report to another physician or to you. Other physicians who are involved in your follow-up care must, however, obtain your premission to release your medical records to him/her and make the request in writing.

Regional Care Association
We want you to be well looked after at home. Therefore, the University Medical Center Freiburg works in conjunction with outpatient centers such as the Care Network PflegeNetz GmbH , and ambulant maintenance service of the Breisgau-Hochschwarzwald and Emmendingen regions. In addition, the University Medical Center has founded the Regional Pflegeverbund (Regional Care Association). This network provides a smooth transition from care in the hospital to outpatient care at home.
The aim of the Regional Care Association at the University Medical Center Freiburg is to facilitate your return home and to optimise conditions for your further recovery. For us, transition of care from the hospital to the home means providing personal and individual consultation as well as high quality care based on coordinated concepts and established standards.

Regional Rehab Network
After your treatment at the University Medical Center Freiburg, it may be expedient to have additional rehabilitation. The University Medical Center Freiburg is aligned with specially selected rehabilitation centers so that you receive the highest quality follow-up care. Partners in the Regional Rehab Network of the University Medical Center Freiburg cover all medical specialties and subspecialties. Transition Care
The Transition Care programme of the Freiburg Cancer Center makes it possible for even the most ill patient with cancer to return home as soon as possible. In collaboration with the oncologist who is responsible for your medical treatment, a mobile team of six nurses and caregivers with experience in the treatment of patients with tumours coordinates your home care. The team organises necessary medications and supplies for you, advises relatives and staff on the special treatment needs of patients with tumours and is involved in symptom and pain control. Treatment Costs
If you have a public health insurance plan, the University Medical Center Freiburg arranges the settlement of your treatment costs with your health insurance company. You will only be invoiced a nominal contribution (see also Additional Payment / Co-Payment ) for your hospitalisation. Your contribution is automatically forwarded to your health insurance company by the hospital.
If you have private insurance, the hospital sends an invoice directly to your private health insurance company or directly to you. In the latter case, you can forward the bill to your insurance company.
