Discharge management
The time of your discharge will be determined by your attending physicians based on your personal recovery process and discussed with you.
In certain cases, further support is required after the end of your hospital treatment in order to secure the outcome of your treatment. Depending on your needs, the social services, the care transition team or the central aid organization will take care of preparing you for discharge and providing care after your hospital stay. Appropriate follow-up care can include, for example, medical or nursing care that is provided on an outpatient basis or in inpatient rehabilitation or care facilities.
The aim of discharge management at the University Medical Center Freiburg is to organize seamless follow-up care for you. To this end, the staff determine whether and which medical, nursing or other measures are required following hospital treatment and initiate these during the inpatient stay. If it is necessary for the immediate follow-up care after the hospital stay, medicines, remedies, aids, sociotherapy and home nursing care can also be prescribed to a limited extent or the inability to work can be determined.
You will be informed about the support options and included in the discharge management planning. If you wish, your relatives or caregivers will also be involved in the consultation.
