Transfer of care
As part of discharge management, the care transition team will support you and your relatives in preparing for outpatient care.
The care transition service at the University Hospital offers you
- Assessment of the current need for help in the areas of: Basic and treatment care, palliative care, domestic care, aids and other support needs in the outpatient area
- Advice on outpatient care options and support in decision-making
- Organization of outpatient post-inpatient care
- Advice on care and health insurance benefits, support with applications
The care transition team coordinates with the other services involved in discharge management, such as social services and the central aid organization.
