After long stays at the hospital, critically ill tumor patients generally wish to be able to return home.
Home care helps you and your loved ones to take this step.

We are qualified nurses and have additional training in oncology, specialized palliative care, and pain therapy.

We are part of the Palliative Care Team SPES and are the specialized outpatient palliative care (SAPV) provider at the Medical Center – University of Freiburg, assigned for organizational purposes to the Department of Palliative Care (in German). In addition to the home care team, a coordinator and a senior physician are also on duty for our patients.

We help you to plan the transition from hospital to home – if necessary and if you so desire, until the end of your life.

• We plan and organize patients’ discharge from the hospital well in advance and arrange for the best possible nursing and medical care at home.
• We enable cancer patients who would otherwise be unable to leave the hospital to live at home.
• We provide a personal point of contact for the entire duration of care.
• We collaborate with hospital physicians, primary care physicians, and specialists, as well as with home care services and other healthcare facilities.
• We advise and support patients, relatives and friends, and caregivers.
• We regularly monitor symptoms.
• We offer support and advice on pain management.
• We perform specialized procedures, such as port care, analgesic infusion pumps, etc.
• We do everything we can to prevent unplanned readmissions to the hospital.
• We support patients during the final stages of life and until the end of life.
• In emergencies, we are available around the clock.
